Booking Terms

These Booking Terms apply to appointment requests, bookings, scheduling inquiries, and related service requests made through beaconspecimen.com, Beacon’s booking tools, or direct communication with Beacon Specimen Collection Services.
 

1. Booking Requests Are Subject to Approval
 

Submitting a booking request does not automatically create a confirmed appointment.

All bookings are subject to:
 

  • service availability

  • service area

  • appointment type

  • staffing

  • required forms or documentation

  • approval by Beacon
     

An appointment is only confirmed after Beacon has reviewed and accepted the request.
 

2. Accurate Information Required
 

You agree to provide complete and accurate information when submitting a request, including:
 

  • contact information

  • service location

  • requested service type

  • requested time

  • program type

  • any information necessary for scheduling or service coordination
     

Beacon is not responsible for delays, cancellations, or incomplete appointments caused by inaccurate or missing information.
 

3. Service Types and Scheduling
 

Service offerings may include:
 

  • urine specimen collection

  • hair follicle collection

  • oral fluid / saliva swab collection

  • venipuncture / blood collection

  • same-day requests

  • after-hours requests

  • on-site or mobile appointments


Availability may vary by location, service type, day, time, and staffing.
 

4. Payment and Invoice Terms
 

Certain appointments may require:
 

  • prepayment

  • a deposit

  • a card on file

  • invoicing after service

  • approved account billing


Beacon may decline or delay confirmation if payment requirements are not satisfied.
 

5. Cancellations and Rescheduling
 

Cancellations and rescheduling are subject to our Refund & Cancellation Policy and any separate client agreement or invoice terms.
 

6. No Guarantee of Specific Outcome
 

Booking a service request does not guarantee:
 

  • eligibility for a specific testing program

  • acceptance by any third-party lab, employer, clinic, legal authority, or program

  • availability of a specific appointment time until confirmed

  • completion of service if required conditions are not met


7. Right to Refuse or Cancel
 

Beacon reserves the right to refuse, delay, cancel, or reschedule an appointment if:
 

  • the service request is outside our scope or service area

  • required documentation is missing

  • payment requirements are not met

  • the location is unsafe or inaccessible

  • the requested service cannot be completed appropriately

  • circumstances arise that affect operations, safety, legality, or scheduling


8. Third-Party Booking and Payment Platforms
 

If bookings, payments, or invoices are processed through third-party platforms, your use of those tools may also be subject to their separate terms and privacy policies.
 

9. Communications
 

By submitting a booking or service request, you agree that Beacon may contact you by phone, email, or other provided contact method for scheduling, follow-up, reminders, service coordination, invoicing, and related business communications.
 

10. Changes to Booking Terms
 

Beacon may update these Booking Terms from time to time. Updated versions will be posted on the Site with a revised “Last Updated” date.
